Much more than usual, now that we've got a snappy build system! This
commit covers
• All .PI functions across all games
• TH02's High Score entry functions
• TH03's shots_update() and shots_render()
• All functions declared in `th04/op/op.hpp`
• TH04/TH05's bb_txt_put_8_raw(), bullet_template_clip(),
player_pos_update_and_clamp(), score_update_and_render(), and
slowdown_frame_delay()
• TH05's reimu_stars_update_and_render(), score_delta_commit(),
stage2_invalidate(), stage2_update(), and space_window_set()

With the script parsing and picture/box rendering functions being as
interleaved as they are, it makes little sense to keep the cutscene
code in more than one translation unit. How would those translation
units even be called? This time, having everything in one file should
beat the few hundred preprocessor-removed lines of code that we end up
adding to each one of the three affected games – and ultimately, you'd
want to merge those systems into one translation unit anyway.

• Comments that describe all lines of code until a blank one are placed
into the lines immediately above
• Comments that describe an entire demarcated block are placed
immediately below the dash row at the top
• In any case, there should be a blank line after the top comment of
a demarcated block, to keep IntelliSense-style systems from applying
the block comment to the first actual line of code…
• …but there shouldn't be one before the dash row at the bottom, where
it'd be redundant.

Might look uglier, but has the advantage of not generating an empty
segment with the default name… *and* the default padding, which will
really come in handy with the following breakthrough.

The regular master.h is big, uncomfortable in C++ mode, and bloated
with all those portability `#define`s that effectively only cover a
small portion of the PC-98-specific code anyway. So, let's gradually
transition to a new smaller header that is more integrated into the
ReC98 codebase, by simply no longer #including `master.h` in new code.
Which also explains its weird place in the root directory. Even though
`libs/master.lib/` contains plenty of game-specific modifications, it
didn't feel right to have anything in this directory refer to types
from `pc98.h`.

28,160 bytes of global data just for a overly large 320×704 bitmap
that allows you to scroll between the general verdict and the
individual end-of-stage scores with the Up and Down keys. That's one
benefit of splitting your game into multiple executables, I guess?

Subpixels with 32-bit X and Y components?! That's new.
And yeah, they still just use 4 bits for the fractional part.
Also, let's try something revolutionary for these declarations that are
only used in a single .ASM dump file, and put them all straight before
the first function they are used in. Then we certainly don't forget to
delete them once we're done decompiling that stuff 🙃
