remove old CI badges and configs
I'm not looking at Coveralls anymore, and getting it working with GitHub Actions on other projects has been a pain, so I'm removing that too. Not opposed to adding code coverage back to CI in the future, though, if anyone else is enthusiastic about some particular tool.
