- scheduler: fix bug in adaptive replication:
if send an unreplicated job to untrusted host,
set both wu.target_nresults and wu.min_quorum to app.target_nresults.

- change get_output_file_path() to look at the result's
xml_doc_in rather than its xml_doc_out;
this is the one that will have <optional/>
